Record and Transcript Requests

Information on transcript orders and additional services is provided below.

  • Walk-in services are not available.
  • Students requesting a transcript who exited a TVDSB school in the past 12 months, contact your last school of registration to request your transcript.
  • Students requesting other student records who exited a TVDSB school in the past 5 years, contact your last school of registration to request the record.
  • Third Parties (including law firms and community partners) requesting official student records on behalf of a student, submit a Freedom of Information request accompanied by a consent form signed by the student.

Important Information

  • Student records are confidential. For students aged 18 or older, records will only be released with written consent from the student.
  • We cannot release any student information over the phone.
  • Incomplete or inaccurate educational and/or biographical information provided as part of your request may cause a delay in the issuance of your records.
  • Processing times can vary and we are unable to provide an estimated timeline.
  • All issues related to course discrepancies, marks, report cards, or graduation status must be handled by the student’s last school of registration, as the Records Centre does not issue report cards, resolve academic discrepancies, or graduate students.
  • Records ordered and not picked up, or returned by Canada Post, are kept on file for 6 months from order date, then confidentially destroyed. Fees are non-refundable.
  • Thames Valley District School Board is committed to providing a healthy and safe work environment for all individuals. The presence of fragrances presents an indoor air quality concern, as individuals may be sensitive to chemical-based scented products (including fragrances). Where possible, please refrain from wearing or using scented products or fragrances when visiting the Records Centre.

Former Thames Valley Students (exited over 12 months ago) - Transcript Request

The Ontario Student Transcript (OST) is the official record of a student's academic achievement at the secondary school level in Ontario.

Important notes regarding the OST:

  • The official document will be in a separate, sealed envelope. It is your responsibility to ask the requester if they need it to remain sealed.
  • To receive a complete/current transcript, your last school of registration or adult learning centre must have been within the TVDSB.
  • To comply with privacy standards concerning the release of personal information, we require individuals 18 years of age and older to submit their own request. 

Submitting the transcript request:

Complete the Online Transcript Request Form to obtain an official transcript.

Note: If you are having issues with this form, please use Google Chrome browser, delete your history, and begin again.

Transcript Fees:

  • Transcript fees are non-refundable.
  • Transcripts are $24.00 for the first copy and $12.00 for each additional copy, ordered at the same time, with a limit of 6 total copies
  • The cost of mailing a transcript via ground mail within Canada and the United States is included in the fee. If you require your transcript to be mailed using expedited mail, please contact records@tvdsb.ca or 519-452-2000 extension 21078 for more information.
Thames Valley Students - OCAS/OUAC Applications

If applying to an Ontario college or university through ontariocolleges.ca, please request high school transcripts online through their site as part of their application process.

Note: If you left school prior to 2002 you will not have an Ontario Education Number (OEN). Enter 9 zeroes in the OEN if you do not have an assigned number.

Additional services:

  • Proof of English instruction while in school (to permit a student to attend English school in Quebec)
  • Proof of school attendance (for citizenship or pension purposes)
  • Proof of a learning disability (e.g. psychological and /or speech and language assessments)

To request these additional services, submit a Student Record Request Form.

Third Party Request for Unofficial Records (school boards, education centres)

If you are requesting unofficial student records (unofficial transcript or graduation summary/credit counselling summary) on behalf of a student, you must complete the Third Party Request for Unofficial Records Form and submit a consent form signed by the student.

Third Party Request for Education Verification

Education verifications confirm a student's graduation status.

Enrollment start and end dates are not provided.

If you are looking to verify the graduation status of a student, you must complete the Online Education Verification Form and submit a consent form signed by the student.
