Students with Life Threatening or Prevalent Medical Conditions
The Ministry of Education has identified four prevalent medical conditions that can result in a medical incident* or a life-threatening medical emergency.
The medical conditions are:

Asthma

Diabetes
-
Epilepsy
-
Risk for Anaphylaxis
* A medical incident is a circumstance that requires an immediate response and monitoring, since the incident may progress to an emergency requiring contact with Emergency Medical Services – 911.
If your child has a life-threatening or prevalent medical condition, we urge you to take a moment to log into the Parent Portal.
Here’s what you need to do:

Log In: Access your Parent Portal account.

Navigate to Medical Information: Click on your child’s home school, then “Access to Information and Completion of Online Forms” and then “Student Medical Information” section.

Complete the Questionnaire: Answer the series of questions provided. This is crucial for ensuring your child receives the appropriate care and support at school.

Submit the Individual Plan of Care (IPOC): Once you’ve completed the short questionnaire, it will trigger the Individual Plan of Care form. Fill it out and submit it electronically to the school.
