Ontario Student Transcripts


THE RECORDS CENTRE HAS MOVED TO THE THAMES VALLEY DISTRICT SCHOOL BOARD DISTRIBUTION CENTRE AT 951 LEATHORNE STREET, LONDON, ONTARIO (see map below).



The Ontario Student Transcript (OST) provides an official and consistent record of the secondary school credit courses successfully completed by a student to achieve a secondary school diploma. Transcripts are required for application to post-secondary institutions and may be required for many other reasons such as employment.

Since the 1999–2000 school year, schools have been required to provide a complete record of students’ performance in Grade 11 and 12 courses. Under this requirement, both successful and unsuccessful attempts at completing Grade 11 and 12 courses must be recorded on the transcript, which will include:

  • all Grade 9 and 10 courses successfully completed by the student, with percentage grades obtained and credits earned;
  • all Grade 11 and 12 courses completed or attempted by the student, with percentage grades obtained and credits earned;
  • all equivalent credits granted through the Prior Learning Assessment and Recognition (PLAR) equivalency process under OSS or through the equivalency process under OSIS;
  • all Grade 10 courses for which the student successfully challenged for credit through the PLAR challenge process, with percentage grades obtained and credits earned;
  • all Grade 11 and 12 courses for which the student successfully or unsuccessfully challenged for credit through the PLAR challenge process, with percentage grades obtained and credits earned;
  • confirmation that the student has completed the forty hours of community involvement;
  • confirmation that the student has successfully completed the provincial secondary school literacy requirement.

Note: Students repeating a course for which they have already earned a credit will earn only one credit for the completion of that course. In addition to recording the number of credits earned, schools may indicate on a student’s transcript that the student has taken a specialized program or a program in a specialized school. For example, students taking a program in one of the arts, technological education, or in a language, may be given recognition on their OST for participating in such a program.

Students who graduated prior to the 1999-2000 school year are not required to complete community involvement hours or the secondary school literacy requirement.

Ordering a Transcript

You can obtain a transcript from the Thames Valley District School Board (formerly Middlesex County, Elgin County, Oxford County and City of London school boards) if the last public secondary school and/or adult learning centre you attended was located within the jurisdiction of the Board.

If the last secondary school or adult learning centre you attended was in another part of the Province of Ontario or in another country, you must contact that school or school board directly. The Thames Valley District School Board does not have access to those records.

The method for ordering your transcript depends on when you graduated/retired from school:

  • Within the Past Five Years, contact the Guidance Department of the last secondary school you attended. Check the SECONDARY SCHOOL LISTINGS.
  • More Than Five Years Ago, complete a Transcript Request Form and submit it to the TVDSB Records Centre via one of the options below. Your transcript request will be processed within 48 hours of receipt of this completed form, as well as the applicable transcript fee and photo ID containing your name and date of birth (e.g. driver’s licence)

By E-Mail:
Scan your completed Transcript Request Form and your photo identification and send them via e-mail as attachments to: recordcentre@tvdsb.on.ca

In addition, the applicable transcript fee must be received by the Records Centre before your request will be processed. This should be sent via regular mail (or Xpresspost if your request is urgent) in the form of a money order which can be obtained at Canada Post offices and banking institutions. Your transcript will be mailed to you unless you indicate otherwise (see Note at the bottom of this page).

By Fax:
Send your completed Transcript Request Form and a copy of your photo identification by fax to the Records Centre at: 519-452-2409. The applicable transcript fee must be received by the Records Centre before your request will be processed. This should be sent via regular mail (or Xpresspost if your request is urgent) in the form of a money order which can be obtained at Canada Post offices and banking institutions. Your transcript will be mailed to you unless you indicate otherwise (see Note at the bottom of this page).

By Mail:
Send a completed Transcript Request Form and a copy of your photo identification to:

TVDSB Records Centre, 951 Leathorne Street, London, Ontario N5Z 3M7.

The applicable transcript fee must be received by the Records Centre before your request will be processed. This should be sent via regular mail (or Xpresspost if your request is urgent) in the form of a money order which can be obtained at Canada Post offices and banking institutions. Your transcript will be mailed to you unless you indicate otherwise (see Note at the bottom of this page).

By Phone:
Contact the Records Centre at 519-452-2000, ext. 21078 to request your transcript. Please be prepared to provide your first and last name (known as in secondary school), date of birth, the name of the last secondary school (and Adult Learning Centre, if applicable) you attended, and the approximate year you retired/graduated from school. You will be advised of a date/time when you can pick up your transcript from the Records Centre located at 951 Leathorne Street, London, Ontario. Parking is available near the entrance of the Records Centre which is located at the front of the building. Bring with you the applicable transcript fee (cash or money order only) and one piece of photo identification. Your transcript will be provided to you unless you indicate otherwise (see Note at the bottom of this page).

In Person:
Bring a completed Transcript Request Form and the applicable transcript fee (cash or money order only) to the TVDSB Records Centre at 951 Leathorne Street, London, Ontario. Parking is available near the entrance of the Records Centre which is located at the front of the building. You will be required to show photo identification when picking up your transcript. Your transcript will be provided to you unless you indicate otherwise (see Note at the bottom of this page).

Records Centre Hours of Operation:

            Monday to Friday, 8:30 a.m. to 4:00 p.m. (except statutory holidays).

Fees: Students currently enrolled in secondary school are not charged for transcripts. (Charges apply only to students who have been out of secondary school for one year or more.) Former students will be charged $10.00 for the first copy and $5.00 for each additional copy, inclusive of regular postage. Any costs incurred for premium shipping (courier, express post, etc.) will be the responsibility of the applicant. Payment must be in cash or money order only (no debit, credit or personal cheques). Money orders must be made out to the “TVDSB Records Centre”.

Note:If you are unable to bring the completed Transcript Request Form to the Records Centre in person, you may designate someone to do so on your behalf by completing the "PICK UP" section of the form as follows:

a) check the "By Other" box;

b) fill in the person’s name on the line beside it;

c) sign your name on the signature line in the box just above that section;

d) fill in the current date to the right of the signature box.

Attach to the completed form a copy of your photo identification and ensure your designate has the correct amount of cash or a money order to pay for the transcript. Your designate will also be required to show their own photo identification when they arrive at the Records Centre.

If you would like your transcript to be mailed and/or faxed to someone other than yourself (e.g. employer or education institution), please complete the “MAIL OR FAX” section of the Transcript Request Form including the name, address and fax number (if applicable) of the person or institution receiving the transcript.
